First head over here and print this Rebate form while it’s available (click on download the form)! While supplies last so you might want to do this sooner then later!
The rebate requires you to purchase 10 select Kellogg’s Cereal through 12/31/11.  Clip your UPC codes off the box and submit them, along with the rebate form to get a $10 PrePaid MasterCard. There is a limit of 5 rebates per household.
